Our Story
Azalea Health and Rehabilitation Center is conveniently located at Select Specialty and Walton Rehabilitation Hospitals. We also accept referrals from Augusta University Hospital and Piedmont Hospital. Working closely with Charlie Norwood Veterans Administration Hospital, we are proud to care for those who served our country in the United States military.
In addition to 24-hour skilled nursing services, we offer onsite physician/nurse practitioner services five days per week, with after-hours on-call services available seven days per week. Our dedicated physical, occupational, and speech rehabilitation staff are in-house and employees of the center.
We offer a host of amenities including spacious resident rooms, free salon services at our Esteem Salon, and a first-class dining experience. We are also proud of our outdoor covered patio space and other scenic outdoor spaces.
We are proud of our partnership with The Joint Commission. Achieving The Gold Seal of Approval from The Joint Commission is an internationally recognized mark of quality and a testament to the quality of care provided at our health center. We are also members of the American Health Care Association (AHCA) and Georgia Health Care Association (GHCA) which is further testimony to the exceptional team, care, and standards we've worked so hard to foster. Azalea Health and Rehabilitation Center has also earned a Bronze Quality Award from the AHCA further commemorating the experience and dedication of our team.

How Does the Admission Process Work?
We will schedule a time to help you complete your admissions paperwork within the first 24 hours. You are welcome to bring any additional questions regarding your insurance benefits and your stay at this time or at any time while you are in the center. We also offer an electronic admission paperwork option that allows e-signature via email if you have a busy schedule or are still at the hospital.

How Do We Establish a Specific Personalized Care Plan?
Your physician communicates medication and treatment orders at the time of your hospital discharge. These orders will be implemented to help provide continuity of care. Upon admission, the interdisciplinary team will work with you and your family to identify care needs, preferences, and goals for your stay. You are a valuable partner in the process. Within the first few days of your stay, you and your family will be invited to participate in a care planning conference to discuss needs, discharge planning, and progress toward goals.
